Under Resolution 105/NQ-CP, the Government’s Action Program implementing Conclusion 210-KL/TW dated November 12, 2025 of the 13th Politburo, the Government issued Appendix II requiring provinces and cities to adjust their health-care organizational structure in the near term.
Appendix II sets a minimum requirement for each province or city to have at least one specialized hospital, one geriatric hospital, or a general hospital with a geriatrics department.
For preventive health public service units, provinces and cities are required to consolidate and upgrade the capacity of the Provincial Center for Disease Control and Prevention. The upgrade must ensure adequate capacity for surveillance, early warning, timely disease control, and proactive organization of preventive health activities in the locality.
For public hospitals (general and specialized) currently operating, provinces and cities are instructed to maintain existing provincial public hospitals and promote socialization where conditions permit.
For district/urban health centers, the general approach is to maintain existing district health centers and convert them into regional health centers. These regional health centers will operate as public service units under the provincial Department of Health after rearrangement, performing functions, duties, and powers in line with Circular 32/2024/TT-BYT dated November 15, 2024, which guides the functions, duties, powers, and organizational structure of regional health centers under provincial authorities.
Based on a review of the scope and effectiveness of existing regional health centers, and according to local health-care needs, the Director of the provincial Department of Health is required to propose and submit to the provincial People’s Committee decisions on whether to maintain them.
Existing regional health centers in a province or city may be reorganized, merged, or dissolved if they are found ineffective. In such cases, functions related to disease prevention, population health, health education, and communication are to be transferred to Commune/Ward Health Posts for implementation. Alternatively, ineffective regional health centers may be reorganized into regional general hospitals to provide basic medical examination and treatment services for local residents.
For district/urban general hospitals, the guidance is to maintain existing district-level general hospitals and convert them into regional general hospitals. After rearrangement, these will operate as public service units under the provincial Department of Health to provide services to residents in the area.
For Commune/Ward Health Posts, implementation is to follow Ministry of Health guidance to ensure basic essential preventive care, primary health care, basic treatment services, and social care for residents in the locality.
